摘要


隨著電子顯微鏡的出現,骨整合的詮釋由光學顯微觀察下所謂的在骨頭與植體之間存在著功能與結構的直接接觸(direct contact)的概念,到在骨頭與植體之間的交界面仍存在著一些amorphous layer,包括proteoglycan及混合一些immature fiber與filament。這凸顯出一個事實:即使被公認親和性相當高的鈦合金,對緊緊相鄰的骨頭而言,也會產生某種程度的異物排斥反應。在本文的第二部份,將綜合第一部份所涉入的材料科學與骨生物醫學,由In vitro experiments到in vivo studies以及human researches的涉入,先從細胞層面來探討軟組織與植體之間的吸附與癒合的機制,再由組織變化來討論骨內傷口的癒合狀況。藉由軟組織與植體的互動,可以了解植體對外部環境的刺激如何建立其保護模式。也藉著熟悉骨頭修復的機制,臨床醫師可以來預測日後植體與骨頭的互動機轉。

With the emerging development of electron micro scope, the disclosure of osseointegration was upgraded from the level of micrometer to the level of nanometer for clinicians. The definition of osseointegration was also switched from the concept of direct functional and structural connection between bone and implant to the findings described as an amorphous interfacial layer with proteoglycans, infiltrated immature filaments and fibers under the observations of electron microscopy. It reveals a fact that there still seems to have some kind of foreign body rejection when bone was next to the highly compatible titanium alloy implant. In the second part of this review, the detail attachment for soft tissue to implant surface will be discussed first based on Bone Biology and previous biologic material, and the healing process of intrabony wound is investigated further based on histological changes. Under the full understanding of interaction between soft tissue and implant, it helps understand how the protective mode of implant is established once facing the outer environment. Based on the exploration of bone biology, clinicians can also predict the possible interaction between bone and implant.
