使用電腦斷層影像評估台灣人族群下顎骨舌孔的位置

The Mandibular Lingual Foramen in Taiwanese Population: Evaluation with CT Image

摘要


因為下顎重建的需要,施行在下顎骨前牙區的手術越來越普遍,然而文獻上有一些術後併發症的報告,顯示在下顎前牙區術前應注意該區域的神經血管構造及分布。下顎骨舌孔(lingual foramen)大多出現在下顎骨舌側正中央,位於頦結節(genial tubercles)的高度或高於頦結節的位置,如果在下顎前牙區的手術不慎將舌側骨板(lingual cortical bone plate)穿通,有可能會傷及舌下動脈(sublingual artery),在少數的情況下會產生危及生命的出血。本篇研究利用電腦斷層影像,分析台灣人下顎骨舌孔的位置與其解剖構造的相對關係。總共收錄30位在本院接受植牙手術前斷層掃描檢查的患者,其中14位為男性,16位為女性,平均44.3歲。排除下顎前牙有缺牙,下顎前牙區若有阻生齒或有病灶,以及有系統性疾病的患者,利用電腦3D軟體重組且斷層掃描影像,在下顎前牙區正中位置的橫斷面上定出舌孔開口,測量舌孔至齒槽骨脊及至下顎骨最下緣之垂直距離,以及下顎骨在舌孔位置之厚度與下顎骨高度並進行統計分析。結果發現每位病人身上都可觀察到舌孔的存在,下顎骨在舌孔位置之厚度為10.78±1.71毫米,舌孔至下顎骨最下緣之垂直距離平均為16.06±1.93毫米,舌孔至齒槽骨脊之垂直距離平均為13.39±3.22毫米,舌孔至齒槽骨脊之垂直距離和下顎骨高度之比率平均為45.0±7.6%,根據本研究,在台灣族群中下顎舌孔約位在下顎骨高度之舌側中間位置。

Various surgical procedures are carried out, and the region is often considered as rather safe in the anterior jaw bones. With the increasing usage of implants and grafting procedures for jaw bone rehabilitation, the number of reported postoperative complaints has been increasing. Previous reports and reviews on neurosensory disturbances and hemorrhage in the anterior mandible demonstrate that the presence of neurovascular structures should be carefully assessed prior to mandibular surgery. The objectives of this study were to evaluate the presence and course of the Iingual foramen of the human in Taiwan on the computed tomography. Material and methods: In this study, total 30 patients (14 males and 16 females) were included. Thay all had dental CT of the mandible for preimplantation evaluation. By the cross-section of the CT image of anterior mandible, the orifice of lingual foramen was defined. The distance of lingual foramen to alveolar crest margin and lower border of mandible and the thickness of mandible were measured and analysed with Mann-Whitney U test and Spearman's correlation analysis. Results: In this study, the thickness of mandible was 10.78±1.71 mm, the distance of lingual foramen to lower border of mandible was 16.06±1.93mm,lingual foramen to alveolar crest margin was 13.39±3.22mm, and ratio of the distance from lingual foramen to alveolar crest margin and mandible height was 45.00±7.6%. The mean distance of BE in male was longer than that in female, which had significant difference. Ratio of AB/BE in male was 48.03±5.2%, and that in female was 42.29±8.4%, which had significant difference. Conclusion: According to this study, the location of lingua1 foramen of mandible in Taiwanese population was approximately at the medium of lingual side of anterior region.
