Mandibular First Molar Distolingual Root and Management Furcation Defects: An Asian Point of View

摘要


牙菌斑引起之牙周疾病的病理機轉可謂相當的複雜,但一般咸認為是受多重因素所影響,其中主要致病原因為細菌性牙菌斑之感染起始宿主免疫發炎反應,隨後因炎性反應擴大,進而破壞牙周結締組織及骨代謝之平衡。在臼齒區域,有許多有關牙齒發育與解剖型態之異常被視為牙周病起始與破壞之局部貢獻因子。因此瞭解臼齒解剖型態及根叉侵犯,對於臨床診斷與治療計畫擬定有深遠之影響,惟有關下顎第一大臼齒遠心舌側牙根對牙周病學之影響所知仍相當有限。本文將著重於探討國人下顎第一大臼齒遠心舌側牙根解剖形態之變異、盛行率、診斷及對牙周疾病之影響及治療策略考量作一文獻回顧。

Periodontal diseases are characterized by the destruction of tooth-supporting structures, which results in a loss of support of the affected teeth. Despite increasing knowledge of the pathological alterations associated with periodontal destruction, it is generally agreed that inflammatory periodontitis is multifactorial and involves many cell types, cell products and cell-cell interactions as well as host susceptibility. This intricate network of interactions is initiated by the presence of complex subgingival biofilms that subsequently trigger host immune responses. A consequence of this is a dysregulated immuno-inflammatory process that may lead to connective tissue destruction and an imbalance in bone metabolism. Several molar developmental and anatomic variations have been regarded as contributing factors to furcation involvement. Knowledge of the anatomy of the oral structures, especially root anatomy, is therefore necessary when diagnosing and treating existing disease or managing potential problems. However, the information regarding the impact of mandibular first molars with a distolingual roots is still limited. The objective of this literature review was to focus on the prevalence of mandibular first molars with distolingual roots and how their presence affects the diagnosis and management of furcation-involved molars with distolingual roots.
