疼痛控握特質與病患使用自控式止痛劑行為之相關性

Relationship Between Characteristics of Pain Locus of Control and Behavior of Using Patient-controlled Analgesia

摘要


本研究目的在探討疼痛控握特質與術後病患之止痛劑行為之相關性;即給予病患衛教與行為提示是否會受到個案本身疼痛控握特質之影響。以台北市一所醫學中心肝膽腸胃外科病房之手術病患為對象,經立意取樣方式,共選取41位患者為研究樣本,於術前給予止痛劑使用之衛教,並於術後48小時中進行行為提示。採用的研究工具包括標準化之自控式止痛使用手冊、疼痛控握量表與病患自控式止痛機;藉以了解病患疼痛控握特質狀況與止痛劑使用次數。研究所得資料以SPSS/Windows進行統計與分析。研究結果發現:本研究個案皆偏向較強之醫護專業控握特質,但止痛劑使用累計次數或劑量與病患四項控握特質皆未達顯著之相關,顯示四項控握特質對止痛劑使用行為的影響有限。本文就上述結果進行討論分析,並提出建議及未來研究方向。

The purpose of thie present study was to understand how the pain locus of control characteristics affects the acceptance of the cues to the pain control. Forty-one sample subjects were recruited at two general surgical wards of a medical center by purposive sampling. Subjects received an one to one, standard and systematic teaching program that focused on how to use the PCA correctly. A pamphlet regarding the safety of using PCA was also provided to each subject. Subjects also received six times standard active prompting that cued them to use the PCA within 48 hours after the operation. The Pain Locus of Control(PLC) scaled and the machine of PCA were used to collect the data. Findings from the study were the followings: (1)Ss had higher tendency on the medical professional locus control to their post-operative pains. (2)The characteristics of pain locus of control had little effect on the cues of using PCA. There were no significant differences among patients with the internal, fate external, medical professional external, and other external locus of pain control to the use of PCA. Implications of findings for nursing research and suggestions are also discussed.
