摘要

接受冠狀動脈繞道手術(CABG)的病人會產生身、心及社會各方面的不適，加強對病人術後健康照護需求的了解與照護，將有助病人手術後疾病的恢復。本研究目的為瞭解CABG病人手術後住院期間健康照護需求狀況、重要程度、滿足狀況及預測因素。本研究對象為台灣南部三所醫學中心及一所區域醫院74位接受CABG手術後病患，以''冠狀動脈繞道手術病患健康照護需求量表''收集資料。研究結果顯示CABG患者術後健康照護需求出現率為81%，其中以心理社會需求最多，其次為訊息需求與生理照護需求，其中以如何預防及處理手術後合併症之需求最多。另外，CABG患者認為生理照護需求最為重要，其次為訊息需求及心理社會需求，其中以「瞭解如何控制疾病危險因子，預防疾病再發」需求最重要，然而只有29.6%病人滿足。健康照護需求不滿足率整體而言為50%。另外，性別、教育程度、社經階層及術後住院天數為影響整體健康照護需求重要性的顯著因素。本研究結果可作為護理人員提供冠狀動脈繞道術患者術後健康照護之參考。

Patients undergone Coronary Artery Bypass Graft Surgery (CABG) often experience physical and psychosocial discomfort. Understanding the health care needs of these patients is helpful for patients’ recovery. The purposes of this study were to examine 1) the caring needs, (2) the importance of caring needs (3) the satisfaction of caring needs, and (4) the predictors of caring needs of hospitalized patients after CABG. Seventy-four subjects were recruited by convenience sampling from three medical centers and one regional hospital in southern Taiwan. A self developed instrument - Coronary Artery Bypass Graft Health Care Needs Inventory was utilized. The results of this study showed that 81% patients reported psychosocial, information and physical needs accordingly. Among these, physical need was regarded as the most important one. , followed by information and psychosocial needs. Among all caring needs, 50% caring needs were unmet. Information needs were the least satisfied one. ”How to prevent and manage postoperative complication” was the most frequent need. ”How to reduce risk factors and prevent the recurrence ” was the most important need, but only 29.6% patints were satisfied. In addition, the best predictors of caring needs were gender, socioeconomic status and length of hospital stay after surgery. These factors accounted for 22.3% of the varianceof total caring needs. The results of this study provide a comprehensive understanding of caring needs for CABG patients and may help nurses to improve the quality of nursing care for patients after CABG.
