多樣感覺剌激對早產兒體重與行為發展之影響

Effects of Multi-Modality Sensory Stimulation Program on Body Weight and Behavior Development of Premature Infants

摘要


本研究目的在探討多種型式感覺刺激措施的介入對早產兒體重與神經行為發展上的成效,採類實驗性設計,以北部某一醫學中心新生兒病房的早產兒,立意取樣符合選樣條件者依出生體重及懷孕週數層化後,採隨機和輪流方式分派至實驗組與控制組,有效樣本實驗組20位、控制組21位,控制組的早產兒依病房常規提供照護,實驗組早產兒除依病房常規提供照護外另在保溫箱內提供多種型式感覺刺激措施,每天三次,一次需時十七~十八分鐘,一週五天,共計十天。在刺激措施介入前一天(Day0)及結束後一天(Day11)做研究前後的測試。結果顯示:一.實驗組的平均體重增加大於控制組且達顯著差異。二.兩組早產兒體重生長曲線有交叉,實驗組曲線斜率大於控制組且達顯著差異。三.神經行為發展測試得分在肌肉張力與動作型態、原始反射及行為反應等三個分測驗得分及總分,實驗主且都比控制組得分較高且在肌肉張力與動作型態分測驗有達統計學上顯著差異,各細項得分以腳躁背屈、拉坐、頭部翻正、定向反應達統計學上顯著差異。本研究結果可說明多種型式感覺刺激措施對本國早產兒確實有幫助。

The purpose of this study was to explore the efficacy of a multi-modality sensory stimulation program on premature infants’ body weight and neurobehavioral development. Quasi-experimental design was adopted for this study. The subjects were premature infants in a newborn ward in a medical center in northern Taiwan. The subjects were selected from premature infants whose conditions were in accordance with predetermined sampling requirements. Subjects were assigned to a treatment group or a control group based on a stratification of gestational age and birthweight, sampling and take turns. There were 20 premature infants in the experiment group and 21 in the control group. The premature infants in the control group were cared for according to regular ward routine. In the experiment group, in addition toward romaine, the infants underwent a sensory stimulation program in an incubator for 17-18 minutes 3 times per day, for 10-day period. Evaluations were done on Day 0 and Day 11. The study results showed: (1) the average weight increase in the experiment group was higher than that in the control group and it was statistically significant. (2) The slope of the weight growth curve in the experiment group was steeper than that of the control group and the difference was statistically significant. (3) In the experiment group, the scores in tone and motor patterns, primitive reflex and behavioral responses of a neurobehavioral evaluation and the sum score were higher than those in the control group. The differences in tone and motor patterns were statistically significant. The scores for sub-items, such as ankle dorsiflexion, pull to sit, head righting and orientation were significantly different. The results of this study showed that a multi-modality sensory stimulation program is helpful for premature infants.
