整合式失智症照護門診
An Integrated Multi-disciplinary Consultation Clinic for Dementia Care
摘要
我國失智症盛行率隨人口老化而逐年攀升。雖然國內提供失智診療服務的醫院不少，失智症照顧者面對日常生活照顧與因應病人問題行為的知識與能力，卻常得不到適時的協助與解答。本文報告成大醫院失智症中心的諮詢門診服務概況。該諮詢門診於2011年1月至2013年12月期間提供失智症及其照顧者共603人次的諮詢服務。在諮詢使用者中，家庭成員照顧者占約九成，其中以女兒及媳婦最多;諮詢的問題以「日常生活」最多，其次為「家庭照護與社會資源」、「問題行為」及「認知障礙」，這顯示台灣失智症照護者的主要需求與不足之處；同時，從三年的諮詢服務經驗得知，雖然失智病人的問題多重且複雜，但他們多數仍可與家人生活，因此失智病人的在宅老化應該被嚴肅地思考。

The prevalence of dementia has increased due to population aging in Taiwan. Although the medical services for dementia are provided in many hospitals and clinics, the needs for caring skills or related knowledge for caregivers are not met. We report a consultation service provided from January 2011 to December 2013 through a clinic for dementia care at National Cheng Kung University Hospital. The service provided 603 consultations, among which 90% of consultation users were family members, mainly daughters or daughters-in-law of patients. The questions raised in the consultation service were 'activities of daily living' as the most frequent, followed by ＂family care and social resources, ＂problem behaviors＂ and ＂cognitive impairments＂. From the experience of consultation service over the past 3 years, we learned that most of the families could live with their dementia patient even though the caring problems were complicated. We suggested that aging in place should be seriously considered for the dementia.
