某醫學中心實習醫師臨床訓練之探析

摘要


醫學生教育和訓練的過程中,臨床訓練是非常重要的一環,其不僅奠定了醫學生臨床知識、技能的基礎,更影響著未來從醫生涯的選擇。本調查的目的,為透過問卷調查的方式,以中部某醫學中心醫學系實習醫師為對象,針對內、外、婦、兒四個科別進行調查分析,期能了解指導醫師的教學成效及學生的學習評量結果,以做為臨床教學訓練計劃修訂之參考。整體問卷的回收率為48%,結果發現,主治醫師及住院醫師的教學評價一般而言均在「佳」左右,其中住院醫師的教學評價內科比外科高,另對住院醫師及主治醫師的教學整體印象中「教學態度」有顯著影響。整體而言,實習醫師學習評量之評價都在「中等度」以上,其中內科primary care的執行成效,外科相較於內、婦、兒科有其限制。另在開放性意見中部分實習醫師認為從事過多的例行性工作,剝奪其他學習機會,亦是臨床訓練的過程中應重視的問題。

Internship is an important part of any medical education and training. It lays the foundation of clinical practice and has a crucial impact on the students' decision-making during their future careers. The purpose of this study was to assess the instructor's teaching effectiveness and students' self evaluation of learning using questionnaires. All interns from a variety of different departments at a medical center in central Taiwan, made up of Internal Medicine, Surgery, OB-GYN and Pediatrics, were enrolled. The results would be used as a reference for the modification of the current clinical internship training program. The overall response rate to the questionnaires was 48%. The data showed that the teaching qualities of attending doctors and residents were evaluated as ”good”. The interns were more satisfied with teaching by residents from Internal Medicine than by those from Surgery. Statistical analysis also showed that teaching attitude had a significant influence on the interns' overall impression of the instructors. In general, the interns' self-evaluation was ”fair”. The implementation of primary care was more restricted in Surgery than in any other departments. In an open-ended question some interns thought that they spent too much time on routine work and this resulted in fewer learning opportunities. This is an important problem and should be taken into consideration when the internship program is modified in the futures.
