台灣醫學教育改革中的“醫學人文”概念與實踐初探

A Preliminary Study of Discourses and Practice on "Medical Humanities" in Taiwan's Medical Education Reform

摘要


近年來,「醫學人文教育」在台灣醫學院校成為一種時代性的「政治正確」企圖重新「把人帶回醫學」,期許新世代的醫療人員能具備更豐富的「人文素養」。本文試圖以台灣「醫學人文教育」為主軸,擬定幾組研究問題,從文獻回顧的歷程描繪台灣醫學教育改革中,關於「醫學人文」的主要論述形成背景、轉變歷程,以及實踐風貌。我們期盼這樣的研究探索,能在各種教改方案研擬之外,開啓台灣醫學教育研究的新取向。本文提出了台灣醫學教育改革中,關於「醫學人文」的主要三個論述與實踐的主軸,包括(1)「醫學人文教育」成為醫學教育改革和通識教育發展趨勢的聯繫,(2)「醫學人文教育」作為醫界社會運動的路徑,(3)「醫學人文教育」與「美感經驗」和「藝術鑑賞」的聯繫。此外,本文提出目前關於「醫學人文教育」改革的三個主要的爭議,分別是(1)「貧穿」與「淨空」的爭議,(2)「一般性」與「特殊性」的辯論,(3)「認知」與「態度」的取捨。

In recent years, ”medical humanities education” has become up-to-date ”political correctness” in medical schools in Taiwan, attempting to ”bring the humanities back into medicine” and to equip the new-generation medical personnel with abundant humanities accomplishment. However, after proposing various discourses of ”medical humanities” and enthusiastically advocating and taking action, we realize startledly that these advocacy and actions gradually lose their vigorous energy as well as more open thinking and discussion. At the same time, these advocacy and actions frequently become formalistic decoration and compliments, which enable the current situation of ”medical humanities” education reform to face the greatest predicament and challenge. In This article, we attempt to discuss in terms of the predicament of ”medical humanities” education reform. By exploring from the major reformers discourses and practice of medical humanities, we intend to illustrate ”medical humanities” in Taiwan s medical education reform, including its forming background, the process of transformation as well as its advocacy and practice. At the end, we expect that in the course of researching we can clarify the major issues and predicaments of medical humanities education reform nowadays, and that we can furthermore make a suggestion of helping us out of the predicament. We are also looking forward that such research can not only be helpful for planning the schemes of education reform, but also can open a brand-new window on medical education study in Taiwan.
