An Integrative Curriculum of Perseverance Enhancement for Medical Students throughout their Early Career: Literature Review and Design Proposal

摘要


Difficult patients or encounters related to medical disputes are essential sources of emotional stress for medical personnel. There is no useful ready-made model to alleviate the burden, and the management of medical disputes requires in-depth understanding and intervention. The best strategies for early career doctor's emotional resilience and perseverance training are still to be studied. An ideal course should be intersected between different grades and extended to the postgraduate hospital teaching courses. This review aims to consolidate current shreds of evidence about the association between physician job stress and physician-patient relationship; to find the possible solution to enhance physician's inner resources through medical education; to propose a feasible curriculum design to actualize this goal. The current knowledge-practice gap, which includes these vital teaching objectives such as treating difficult patients, physician-patient communication, preventing medical disputes, and emotional resilience, need to be integrated. Based on this review, we proposed an innovative vertical integration of training courses to improve medical students' emotional resilience from lower grades to the resident physicians. Stepwise and systemic exposure to designed difficult patients or encounters can further enhance system-based medical practice. Such courses could provide a fresh learning experience in medical education to improve physician-patient communication quality and enhance the perseverance when facing the challenging medical environment.
