Using an Innovative Problem-oriented Medical Record Contest to Assess Students' Medical Record Writing

Purpose: To use a simulated case and a marking scheme in a problem-oriented medical record (POMR) contest to assess medical students' competencies in writing medical records. Methods: POMR contests were held in 2019 and 2020. A structured marking scheme for checking critical points in POMR writing was established. A file containing focused physical examination (PE) findings for commonly-encountered diseases was introduced into the 2020 POMR contest. The POMR writing was assessed by two raters, and their respective scores were correlated with each other using Pearson's correlation. The students' performance in the 2019 contest was compared with that in the 2020 contest with a chi-square test. Results: The correlations between the rating scores of the two raters were 0.901 and 0.941 in the 2019 and 2020 contests, respectively. In the 2019 contest, more than 30% of the students did not record the PE findings for differential diagnoses and comorbid disorders. Introducing the focused PE findings in the 2020 contest improved the records of PE findings for the main problem, differential diagnoses, and comorbidities. In addition, neglecting weak family support, incomplete assessment of the problem, and premature diagnostic closure of the main problem and comorbidity were found to be the common flaws in the students' POMR writing. Conclusions: Our POMR contest discovered the inadequacies of students in writing medical records. Our structured marking scheme can be used as a summative assessment for POMR writing. Applying focused PE findings can improve the POMR writing.
