The Use of Misoprostol to Control Postabortion Uterine Bleeding
使用misoprostol來控制流產後的出血
摘要
目的：試驗以直腸置放misoprostol是否對流產後子宮的出血有所助益。 病例報告：我們報告一懷孕約13週婦女，進行子宮搔刮術時出血甚多，同時取出很多的胎盤組織，此時，以為子宮內容物已排空。但患者在恢復室時仍有陰道出血，經使用oxytocin注射無效，然後在肛門塞入misoprostol 400μg，一小時內出血漸漸停止，但清除陰道血塊時卻發現整個胎兒和小部份胎盤，2小時後患者一切正常才出院，估計出血約350ml。 結論：直腸置放misoprostol可以幫助控制產後出血。至於以肛門使用misoprostol 來幫助晚期第一孕期流產後的出血，必需更多試驗才能評估效果、劑量和副作用。

Objective: To test whether the rectal administration of misoprostol can contribute to control the postabortion uterine bleeding. Case Report(s): We present a pregnant woman who underwent an uterine curettage at 13 weeks’ gestation. Unfortunately, the uterine bleeding was noted despite the administration of an oxytocin infusion. Thus, 400μg of misoprostol was given rectally, and the uterine bleeding gradually ceased over the ensuing 60 min. Conclusion(s): The use of rectal administration of misoprostol might contribute to control post-abortion uterine bleeding. A larger study is needed to better assess the efficacy, dose, and side effects of such a medical practice.
