Objective: To report and review the management of a rare primary stramal sarcoma of the ovary. Case Report: We report a rare type of ovarian malignancy that occurred in a 60-year-old female. She visited our department due to lower abdominal discomfort. Ultrasound and abdominal computed tomography showed a heterogeneous mass over the left adnexa without ascites or lymphadenopathy. During surgery, we found an irregular ovarian tumor arising from the left adnexa with severe adhesion at the cul-de-sac. Frozen sections indicated a malignancy, so debulking surgery was performed. The final histopathologic findings showed a lowgrade stromal sarcoma of the left ovary with omental and uterine metastasis. Adjuvant chemotherapy with a combination of70 mg/m2 epirubicin, 50 mg/m2 cisplatin, and 6 g ifosfamide was given. Unfortunately, lymph node metastasis and tumor recurrence were noted at the end of the third course of chemotherapy. Conclusions: The prognosis for sarcoma is poor and these tumors are always radio-resistant. Adjuvant chemotherapy with doxorubicin and other agents may provide some response. Unfortunately, we found multiple intra-abdominal metastases in the patient. The disease course was 10 months. [Taiwanese j Obstet Gyneco/2004; 43 ( 2 ):110-112]
