Jet Findings After Extracorporeal Shock Wave Lithotrispy for Ureteral Stone: Report of a Case

輸尿舌結石接受體外電震波碎石術後之輸尿管噴射表現:病例報告

摘要


用超音波來偵測膀胱內的輸尿管噴射以評估輸尿管阻塞或傷害的情形,已於多年前被提出。隨著內視鏡手術於婦科領域的廣泛應用,輸尿管傷害的案例也隨之增加,我們常規用輸尿管噴射來評估所有接受過較複雜內視鏡手術的患者。本文報告一接受複雜內視鏡手術的婦女,術後接受輸尿管噴射超音波來評估輸尿管,意外發現有單側輸尿管結石,患者於泌尿科門診接受碎石手術,同時也陸續於婦科門診作手術後的追蹤。我們發現膀胱內的輸尿管噴射於碎石後的不同時期,有不同表現,此或許值得進一步研究,以可提供輸尿管結石之治療評估參考。

A female patient received laparoscopic total hysterectomy for uterine leiomyoma. Sonographic examination over bilateral ureteral jets, bilateral kidneys and gynecologic pelvis was done during a routine examination for all high-risk ureteral injury patients in our study. A dilated left renal pelvis, about 2.5 cm in diameter which mimicked a postoperative ureteral injury, was detected. After intravenous pyelography evaluation, a stone was found in the left ureter. The patient received extracorporeal shock wave lithotrispy (ESWL). Serial sonography and kidney-ureter-bladder X-ray films were done during follow-up. Initially the left ureteral jet was absent after ESWL but reappeared two weeks after ESWL. Thus, routine ureteral jet and bilateral renal sonography for patients who have undergone advanced laparosocopic surgery is a simple and effective method for detecting urinary track injury and ureteral stones. It can also play an important role in the follow-up examination of patients with ureteral stones after ESWI.
