摘要

本文報告一例很罕見的馬尾副神經節瘤，一位74歲男性，罹患長期間歇性下背痛，為期20年之久。某日，突然發生急性尿液滯留，而後下肢無力，且臥病在床無法行走。病患的腰椎脊髓攝影及磁振造影顯示：腰椎第二和第三節間有一卵圓形硬膜腔內腫瘤。 手術發現一卵圓狀腫瘤附著於病患的終端絲。病理組織學和免疫細胞學檢驗顯示是一馬尾副神經節瘤。本文報告此一病例，顯示位於脊髓管腔內的副神經節瘤，是造成馬尾症候群中一種罕見且可治癒的疾病。

We present a patient with an unusual paraganglioma of the cauda equina region. This 74year-old male had chronic low back pain for about 20 years. Sudden onset of urinary retention, and weakness of bilateral lower limbs, and subsequent confinement to bed occurred on the same day. Magnetic resonance imaging of lumbar spine revealed an intradural oval mass at the level of his 2nd and 3rd lumbar vertebrae. Operation disclosed an encapsulated mass attached to his filum terminale. Total extirpation of the mass lesion was performed. Histopathological examination showed a paraganglioma. There has been no recurrence during 2 years of follow-up. This kind of tumor is rare, but treatable, and can be the cause of cauda equina syndrome.
