Correlation of Thallium-201 Mammoscintigraphic Results with P-Glycoprotein, Ki-67, and P53 Expression in Breast Cancers

摘要


目的 乳癌病人手術前的化療已經越來越普遍,因此了解化療之抗藥性對於發展適當之化療療程非常重要。癌細胞上Ki-67、P-glycoprotein或p53之表現於化療抗藥性有相關性。文獻上只有一篇初步研究指出鉈-201乳房閃爍造影可於體外非侵入性地檢測Ki-67的表現。但沒有任何有關鉈-201乳房閃爍造影與乳癌上P-glycoprotein或p53表現相關的研究。因此,本研究之目的在於比較鉈-201乳房閃爍造影發現與乳癌上Ki-67、P-glycoprotein或p53之表現之相關性。 方法 二十四位乳癌病患於手術或切片前進行鉈-201單光子電腦斷層造影。吾人利用免疫組織化學分析對乳癌檢體之切片進行分析。並計算鉈-201乳房單光子電腦斷層造影上之腫瘤對背景比值與免疫組織化學分析之結果相比較。 結果:11位陽性Ki-67表現比13位陰性表現之乳癌病患有明顯較高的腫瘤背景比值(4.26 ± 0.41對2.25 ± 0.03,p<0.05);然而,腫瘤對背景比值於陽性及陰性表現P-glycoprotein或p53乳癌患者並無明顯差別。 結論:本研究顯示利用鉈-201乳房單光子電腦斷層造影計算之腫瘤對背景比值可用於決定乳癌上Ki-67之表現,但與P-glycoprotein或p53則無相關性。

Objectives. Adjuvant and neoadjuvant chemotherapy is increasingly being used in the treatment of breast cancer patients. Determining the mechanisms of drug resistance is imperative for the development of rational therapeutic strategies. The expression of Ki-67, P-glycoprotein (Pgp), or mutant p53 (p53) involves the resistance to cytotoxic drugs. Only one preliminary study has tried the clinical use of thallium-201 (201Tl) mammoscintigraphy for noninvasive and in vivo determination of Ki-67 expression. However, no association of 201Tl mammoscintigraphic findings and Pgp or p53 in human breast cancers was studied. Therefore, the aim of this study was to compare 201Tl mammoscintigraphic findings with the expression of Ki-67, Pgp, and p53 in human breast cancers. Methods. Twenty-four patients diagnosed with infiltrating ductal breast cancer underwent mammoscintigraphy with thallium-201 (201Tl) single photon emission computed tomography (SPECT) before surgery or biopsy. Protein expression was investigated by immunohistochemical analyses on multiple nonconsecutive sections from surgery or biopsy specimens. Tumor to background (T/B) ratios calculated by 201Tl SPECT mammoscintigraphic findings were correlated with protein expression determined by immunohistochemical analyses. Results. The T/B ratios were significantly higher for tumors in 11 patients with positive Ki-67 expression than for those in 13 patients with negative expression (4.26 ± 0.41 and 2.25 ± 0.33, p < 0.05). However, differences in T/B ratios between the patients with positive and negative Pgp or p53 expression were not found in this study. Conclusions. Our data confirmed that T/B ratios calculated from 201Tl SPECT mammoscintigrapy are useful for determination of the presence of Ki-67 expression in breast cancers, but no significant relations between T/B ratios and Pgp or p53 were found.
