影響醫院護理人員自覺肌肉骨骼疼痛症狀之相關因素
Factors Affecting Musculoskeletal Disorders among Hospital Nurses
摘要
目的 調查醫院護理人員自覺肌肉骨骼疼痛症狀之比例及其影響之相關因素。 方法 針對686名及861名護理人員分別來自中部某區域教學醫院及醫學中心的護理人員，研究工具主要參考修正過北歐肌肉骨骼傷害問卷，包括個人基本資料、工作環境影響因素及身體自覺肌肉骨骼疼痛情形。 結果 兩所醫院護理人員之間巷回收率分別為82.7%及92.3%，兩所醫院護理人員之頸部、下背部與肩部疼痛盛行率均超過五成以上。經由邏輯斯複迴歸分析，控制其他變項：性別、年齡、工作年資、婚姻、吸菸、生育史、身體質量指數、醫院別後，得知影響護理人員九大部位肌肉骨骼疼痛的變項，包括工作生活品質差者較工作生活品質佳者罹患肌肉骨骼疼痛有16-3.0倍之危險性，工作不滿意者是工作滿意者的1.7-2倍之危險性，工作負荷沈重者是工作負荷適度者的17-29倍之危險性。 結論 護理人員工作生活品質、工作負荷量、工作滿意度與自覺肌肉骨骼疼痛症狀之比例均呈劑量與效應關係。因此，若能減少其工作負荷驕，以提昇其工作生活品質和工作滿意度，應可改善其肌肉骨骼疼痛的症狀。

Purpose. To investigate the factors affecting musculoskeletal disorders in nurses from two hospitals. Methods. A total of 1547 nurses from one teaching hospital and one medical center in Taichung were interviewed using a modified standardized Nordic Musculoskeletal (NMQ) questionnaire. The NMQ contained questions regarding the demographic information, workplace condition and musculoskeletal disorders. Results. Response rates of participants were 82.7% (567/686) from the medical center and 92.3% (794/861) from the teaching hospital. More than 50% of musculoskeletal disorders were concentrated in the neck, shoulder and low back. Logistic regression revealed that musculoskeletal disorders were correlated with work-related quality of life (WRQOL) and intensity of workload based. Nurses who reported having a poor WRQOL were at higher risk (OR=1.6 to 3.0) of developing musculoskeletal disorders compared with those who reported having a good WRQOL groups. Furthermore, nurses with heavy workloads were at higher risk of developing musculoskeletal disorders (OR=1.7 to 2.9) than those light workload. Conclusions. The severity of musculoskeletal disorders is a dose-dependent with WRQOL and workload. Modification of work process in hospitals will decrease the risk of developing musculoskeletal disorders among nurses.
