摘要

本病例為－25歲骨性三級咬合不正同時伴隨有下顎骨歪斜之成年患者。以傳統矯正合併正顎手術的治療方式，來達成改善顏面外觀與咬合異常之問題。不同於傳統治療方式－正顎手術前需進行一段時間的齒列矯正治療已達成去代償(decompensation)的做法；取而代之的是，先進行正顎手術再接著進行後續的齒列矯正治療。整個療程為期一年，治療結果除獲得對稱的顏面外觀之外，亦獲得理想的咬合情形。

The case report is presented on an adult patient who had surgical-orthodontic treatment to solve her skeletal Class Ⅲ malocclusion with mandible deviation. Conventional approach of surgical-orthodontic therapy requires a varied period of pre-surgical orthodontic treatment, the most important work to do is dental decompensation. The case was treated by surgical-first approach with minimal pre-surgical orthodontics, i.e. most of the detail orthodontic works (leveling, alignment and interdigitation) were done after orthognathic surgery. The total treatment time was 12 months. The facial symmetry and optimal occlusion were achieved.
