以雙顎正顎手術搭配上下複合體旋轉術式改正骨性三級關係及顏面歪斜

摘要

對於嚴重骨性三級關係之患者，雙顎手術搭配矯正治療往往能達到較佳的顎間關係以及美觀的改善。相較於傳統手術設計，對於顎間關係差異較大的患者，也可利用上下顎複合體搭配旋轉的概念，達到較佳的外觀。本病例報告為一骨性三級關係患者，藉由上述的手術設計概念，改善中臉部凹陷、下顎戽斗的情形，縮短下顏面高度以及減少下巴突出程度，達到良好的顏面垂直比例以及術後穩定性。上下顎複合體合併旋轉咬合平面的手術設計方式，相較傳統術式，更能達到理想的外觀改善程度。（J. Taiwan Assoc. Orthod. 24（1）: 35-45, 2012）

The double-jaw surgery is frequently desirable to correct skeletal discrepancies and obtain better esthetic results for patients with severe skeletal Class III deformities. Instead of standard surgical principles, an alternative rotation of the maxillo-mandibular complex (MMC) is frequently required for patients with extreme degrees of skeletal discrepancies to achieve optimal facial esthetics. As shown in the case report, the clockwise rotation of MMC was planned for the adult male patient with skeletal class III that resulted in increasing the fullness of paransal area, reducing the lower anterior facial height and decreasing the chin projection by 14 mm. The proper facial proportion, optimal facial esthetics and stable surgical results were achieved in this case. The rotation of the MMC is an accepted surgical design and should be considered for patients that the conventional procedure will result in less than optimal esthetic outcome. (J. Taiwan Assoc.
