非拔牙之上顎犬齒錯位矯正治療病例報告
A Case Report of Non-Extraction Treatment for Maxillary Canine Transposition
摘要
犬齒錯位是口腔內常見的牙齒錯位，主要發生在上顎犬齒及第一小臼齒處，其發生率約為0.135～0.3%。男性及女性發生率大致相似，單側發生機率高於雙側，左側發生概率高於右側，上顎發生機率高於下顎。雖然犬齒錯位的機制尚未有定論，但推測與遺傳、乳犬齒的延遲脫落，骨性病變（例如囊腫）以及外傷有關。本病例中病患就診時為21歲女性，主訴為右上犬齒及第一小臼齒完全錯位，右上犬齒之牙冠及牙根已完全位於第一小臼齒及第二小臼齒之間，且位於頰側，口內其餘牙齒排列大致整齊，且病患側面外觀良好，考量如將犬齒牽引回正確位置，在牽引過程中可能造成犬齒及第一小臼齒之牙周破壞，因此治療計畫為不拔牙之矯正治療，將第一小臼齒取代犬齒，犬齒取代第一小臼齒，將牙齒排列整齊，達成良好咬合，並且維持目前側面外觀。

Tooth transposition is a rare but disturbing problem for patients in esthetic and functional aspects. In this report, we describe a case with unilateral transposition of the maxillary canine and premolar. The patient received full mouth non-extraction orthodontic treatment with upper 1st premolar substitution for upper canine. Crown morphology modification and gingival plastic surgery for harmonious anterior esthetics and function were performed. Patient was satisfied with the treatment outcome.
