Effect of Steroid Solutions on Surface Tension Properties of Survanta

Tracheal instillation of glucocorticoid/surfactant mixture may be beneficial for reducing pulmonary inflammation with few systemic side effects. However, the effect of glucocorticoids on the surface tension lowering properties of a surfactant has not been fully evaluated. The main purpose of this study was to evaluate the influence of betamethasone (Beta) and budesonide (Bude) solution on the surfactant biophysical properties of Survanta(superscript ®) (Survanta, an organic extract of bovine lung surfactant). Static surface tension properties of Survanta and test solutions were evaluated in vitro by a Fisher surface Tensiomat at 37℃ (n=3). The results showed that the surface tension of Survanta was 40.8±1.5 dynes/cm, that of Beta solution was 57.6±0.7 dynes/cm and that of Bude solution was 47.0±0.4 dynes/cm. Dose effects of commercially available Beta and Bude solutions on the surface tension of Survanta were evaluated (n=5). Direct addition of Beta to Survanta significantly changed the surface tension at high concentrations. Below the concentration of 400 μl Beta/4 ml Survanta (1.6 mg/kg) the surface tension lowering properties of Survanta were not significantly altered. Between dosages of 500 and 1500 μl of Beta solution (2 mg to 6.0 mg/kg) there was a gradually rising trend in surface tension. Bude significantly changed the surface tension of Survanta, even at a low concentration of 50 μl/4 ml Survanta (p<0.001). We concluded that Beta, within the clinical recommended dosage range, did not significantly alter the surface tension properties of Survanta.
