本個案報告描述一位七歲六個月之學齡期第一型糖尿病病童及其主要照顧者面對初次診斷糖尿病的護理經驗。筆者於2005年1月19日至2005年1月26日照護期間,運用健康信念模式並藉由觀察會談、身體檢查等方式收集資料,歸納出個案主要問題有家庭運作過程改變、自我照顧知識及技巧不足、潛在危險性傷害/低血糖等三個問題。透過糖尿病衛教師的傾聽、陪伴、加強認知、反覆練習及提供諮詢,幫助病童及主要照顧者理性面對問題,進而學會相關照護技巧。於護理過程中可見病童生長發育已達理想體位,全家人都在學習一同面對疾病,並漸進式幫助病童學會獨立及自我照顧,以因應未來青少年期發生親子衝突影響疾病控制。
This study describes the nursing experiences of helping a school-age child and his primary caregiver to coping with the child's initial diagnosis of type I diabetes. From January 19, 2005 to January 26 2005, the author applied Health Belief Model and collected related information by observation, interview and physical examination to assess the client. The major problems identified were as follow: change of family process, inadequate knowledge, risk of injury and hypoglycemia. By listening, accompanying, providing intensive coaching to the patient repeatedly, CDE (Certified Diabetes Educator) assisted the patient and his primary caregiver control the patient's body weight with positive attitude and learn the necessary nursing skills. The patient reached the desired growth rate gradually, and family members learned how to cope with the disease. Meanwhile, family members helped the patient to learn independence self-care, in order to prevent paternity conflict reduce the possibility of parent-child conflict in the future during the adolescence
