提昇門診癌症病患攜帶式化療衛教專案
Promotion of Health Care Education for Outdepartment Cancer Patient: The Infusion Pump for Chemotherapy Project
摘要
攜帶式化學治療爲腫瘤病患帶來方便性，也引來了一些居家問題，完整的衛教指導能降低病患焦慮，並提升其自我照顧能力及生活品質，本專案旨在建立攜帶式化學治療病患衛教作業標準，提升護理人員專業護理衛教服務，進而提升病患居家生活品質。專案小組於現況分析發現護理人員對攜帶式化學治療病患衛教不一，因此於2005年4月評量30位初次接受攜帶式化學治療病患，發現其護理衛教完成率僅有63.7%。專案小組透過討論分析及文獻查證，確定問題及目的後，執行專案改善如下：一、建立攜帶式化學治療衛教標準與衛教輔助工具，包含制訂攜帶式化學治療「衛教標準作業」、「衛教指導單張及記錄單」、「衛教輔助工具模型、圖片及衛教光碟」。二、舉辦教育訓練，增強護理人員對攜帶式化學治療衛教之專業知識。經專案改善並評值其成效，發現護理人員其護理衛教完成率可達99%。因此透過行政結構、人員訓練及監測來改善，確能有效的提昇護理衛教品質，確保病患優質的照護。因此建議將化療衛教標準納入護理人員教育訓練，且護理衛教應持續監測評值，以維護品質。本專案首創攜帶式化學治療衛教輔助工具，已作爲他院標竿學習。

The infusoion pump brings the convenience to the patients. But it also comes out with some home problems. However, a full health education can reduce patients' anxiety and raise self-care abilities and quality of life. Accordingly, the purpose of improving the patient service for chemotherapy is to reduce the number of problems which have been encountered. Like build up a health care education standard, increase the quality of life to the patients. Analysis of the present situation revealed that there is a gap on standard protocol for the use of the infusion pump for chemotherapy. Nursing staffs have no identical in health care contents .in addition, this study investigated 30 cases for using the infusor pump for chemotherapy in April, 2005 and the results showed that the achievement rate in health care education for the nursing staff was only 63.7%. An in-depth analysis of the problem and a review of the literature revealed several improvements 1. To devise standard protocol for health care education in general and specifically for the use of the infusor pump for chemotherapy. This included making the infusor pump the 『standard treatment care』 for chemotherapy、maintaining a 『single sheet and a list of instructions for nursing staff』、creating 『instructional models and diagrams or charts of priorities in health education』、 recording a 『CD-ROM of health education instruction』 2. Scheduling education training programs to help the nursing staff become more knowledgeable about the use of the infusor pump for chemotherapy. By following the implementation of the above protocol, the achievement rate in health care education for the nursing staff increased to 99%. Therefore, the results demonstrated that investigation of the cases through analysis and implementation of the instruction plan effectively raised the level of health care education for the nursing staff and guarantee patients’ right offering high quality care.
