醫療機構作業外包管理之文獻回顧1998-2008

摘要


多數醫院爲控制成本,並兼顧醫療品質,常採用合約管理(Contract Management, CM)的經營策略,即外包(outsourcing)。故本文以文獻回顧方式探討醫療機構作業運用外包的項目、可能有的風險、管理方式與影響外包績效的因素。綜合1998-2008年於各期刊發表的28篇醫療機構作業外包管理相關研究,歸納出六個主題,分別爲:外包管理模式、外包考核之管理機制、外包品質與安全管理、外包績效、影響外包決策因素及外包趨勢。且檢視這些研究不足之處,期望藉此提出未來醫療作業外包管理研究之可行方向,並將有助於此研究議題的內涵,做爲醫院護理行政主管之參考。

There is an increasing trend for hospitals to implement contract and outsourcing management for the purpose of reducing operating costs without sacrificing quality of medical services. Studies were made to identify outsourcing opportunities, assess potential risks, and investigate ways to manage outsourcing contracts. Literature review for 28 articles about outsourcing management in medical institutes are published from 1998 to 2008. Six major themes were identified Outsourcing management , quality of outsourcing, advantage of outsourcing, item and position of outsourcing, evaluation of outsourcing performance, and influence of outsourcing decision making. Some deficiencies identified which may provide the basis for future studies. This paper is to share learning from the study with managers of nursing department in hospitals.
