一位攝護腺肥大患者行經尿道內視鏡攝護腺切除術之手術全期護理經驗

本篇個案報告係探討一位攝護腺肥大病患接受經尿道內視鏡攝護腺切除術之手術全期照護經驗。筆者於護理期間應用生理、心理、社會及靈性各層面進行整體性護理的評估，以觀察與會談技巧收集資料，個案主要呈現的護理問題爲：1.手術前後之焦慮，2.術中高危險性傷害，3.術中高危險性體溫改變及4.術後特定知識缺失。在手術全期護理過程中，筆者藉由主動關懷、傾聽和同理心以了解造成個案健康問題的原因，並適時澄清以增進個案對經尿道內視鏡攝護腺切除術的了解，且針對個案身心健康問題需求提供個別性護理，並在手術各時期主動提供適當的照護與技能，協助個案面對健康問題，使其於手術住院期間獲得整體性護理。藉此護理經驗的分享，冀能提供日後手術室護理人員照顧類似病患之參考。

This article reports a pen-operative nursing experience of caring for a surgical patient with prostatic hyperplasia. The patient underwent the removal of the prostate gland via the urethral tract. During the nursing period, the author completed an integral assessment in physical, psychological, social, and spiritual aspects, respectively. Data were collected through patient observation and interview. The identified four major nursing problems were (1) anxiety secondary to surgery, (2) a high risk for surgical injuries, (3) a high risk for altered body temperature during surgery, and (4) lack of knowledge during post-operative care. Through active caring and listening with empathy, the patient was provided with knowledge of the surgical procedure. The patient's concerns about physical and psychological needs were clarified. We hope that this report can be used as a reference for nurses taking care of surgical patients undergoing removal of the prostate gland.
