摘要

本文為照護一位乳癌病人，接受乳房改良性根除術之手術全期護理經驗，照護期間自2010年4月14日至2010年4月22日，利用會談、直接照顧等方式蒐集資料，依歐倫（Oren）護理照顧理論做整體性評估及手術全期照護模式，確認個案有：手術前焦慮、手術中潛在危險性情境傷害、手術後疼痛等護理問題。運用歐倫（Oren）護理照顧理論，透過與個案及家屬共同討論手術護理計畫，給予術前訪視，安排醫療團隊、院牧人員予以術前禱告；術中注意病人安全、舒適和無菌技術；術後減輕急性疼痛和預防麻醉合併症的發生，使個案順利完成手術治療及執行後續治療計畫，期望藉此護理經驗，提供臨床照顧類似病人之參考。

This report describes a nursing experience for a breast cancer patient who recently underwent modified radical mastectomy. The data were collected by means of interview and direct patient care. The nursing care period was from April 14 to April 22, 2010. Based on Orem's nursing theory, integral nursing assessment and peri-operative nursing care model were applied. Identified nursing problems included pre-operative anxiety, potential risk of intra-operative injury, and post-operative pain. The operative nursing care plan was discussed with the patient. A pre-operative nurse's visit for patient education and the chaplain's pray for emotional support were arranged. Attention was paid to patient safety, comfort, and sterile techniques during operation. Post-operative care included pain relief and prevention from the complications secondary to surgery. The author expects that this care experience can be a reference for other nurses caring for the patients receiving mastectomy.
