The incidents relate to incomplete and inaccurate nursing reports during shift changes not only impact patient safety, but also affect nurses' morale with the medical teams' perception toward nursing competency. The aim of this project is to improve completion and accuracy of nursing reports exchange during shift changes for patient safety and quality of care. The strategies adopted were establishing the guidelines for nursing reports during shift changes, holding professional seminars, developing an online self-learning program, including nursing reports exchange as a of the education curriculum, and forming a monitoring system. With the implementation of this project, the rate of completion and accuracy nursing reports during shift changes were improved from 63% to 91%. This project expanded into other medical units also having good results and promoted the relationship among the entire health care team. The aim of this project was achieved.
