照護一位老年急性混亂病患的護理經驗

摘要

本文係描述一位因急性混亂入院老年病患的護理經驗；照護過程運用Gordon十一項健康功能型態進行評估；發現主要護理問題有急性混亂與主要照顧者之角色緊張；提供急性混亂護理措施：安全維護、情緒穩定、去除導因及協助恢復認知功能；主要照顧者則給予急性混亂相關知識、技能、預防策略和照顧技巧之指導，並緩解其緊張情緒；經上述護理處置後，個案急性混亂情形漸趨穩定並出院，而照顧者也藉由學習過程，漸能掌控急性混亂的預防及照護技能，並改善個人生活品質。期望藉此照護經驗分享，能作為臨床護理人員照護此類個案時的參考。

This article describes a patient with acute confusion in hospitalized elderly nursing experience; care process using Gorden eleven functional health assessment; found that the main health problems are two: one for cases of acute confusion; two for the caregiver Role strain; The nursing interventions given to case included: security maintenance, emotional stability, removal of causes and help to restore cognitive function ; and interventions for primary caregiver are include knowledge, skills, prevention strategies and care skills with acute delusion, and alleviate his emotinal tension; through all nursing interventions, the cases of acute confusion subsided and the situation stabilized and discharged, the caregiver through the learning process also, increasingly able to grasp the prevention of acute confusion and care skills, and improve the personal quality of life. Expected to share this experience of care for clinic nurses as a reference when taking care of these cases.
