長期照護的複雜度是因(1)服務對象帶有多重慢性疾病,(2)需跨科別和跨專業合作提供照護,(3)工作場域為多型態,(4)服務內容有生活支持和醫療照護;並且是勞力密集,低薪、工時長、吃力、又不討好的產業,需要建立教學策略培育不同專業人才參與,方能有效益、滿足實際需求。目前長照2.0,社區「照顧管理督導」、「照顧管理專員」、「個案管理師」需2年工作經驗。但兩年的實務經驗不足以應付社區複雜個案的評估,確立健康問題,提供社區資源和照護計畫。所以長照教學策略必須加強複雜居家個案的實務經驗,方能訓練有素質和能力的評估和訂立計劃者。建議新課程模組有(1)基本知識與技能:適合護理師及資深照顧服務員學習;(2)跨專業角色釋放課程:適合各專業師級人員;(3)管理層級課程:適合照護管理督導、照護管理專員、個案管理師。綜觀當務之急是變更教學模組,因應高齡社會需求,兼顧照護品質。
The complexity of long-term care was caused by: (1) cases of residents with multiple chronic diseases, (2) requirement of care provided through cross-disciplinary and interdisciplinary professional cooperation, (3) professions are multidisciplinary categorized, (4) service content includes life support and medical care. Additionally, labor-intensive, low-pay with long working hours, hard work and unpleasantness are all part of the issues. It is necessary to establish teaching strategies to foster the participation of various professionals in order to be efficient and meet actual requirement. At present, the Long-Term Care 2.0 requested "the superintendent of care managers", "care manager" and "case manager", they only required 2 years of work experience. However the actual practical experience required is far beyond the assessment for establishing the health issues, providing community resources and care plans for complex cases in the community. Therefore, long-term care teaching strategies must be strengthen for practical experience of complex cases at home in order to train those with the quality and ability to evaluate and plan. It is suggested that the new curriculum modules to include: (1) basic knowledge and skills that suitable for nurses and senior care attendant, (2) release the role of interdisciplinary professionals who are suitable for multidisciplinary professional teachers, (3) management level courses: suitable for care manager, the superintendent of care managers and case manager. It is imperative to change the teaching module to meet the needs of the elderly and to take care of the quality of care.
