運用輔助療法照顧一位卵巢癌復發患者之護理經驗

摘要

本文探討一位卵巢癌復發之個案，面臨化療後經歷身體不適及對治療失去信心，產生無望感之護理經驗。護理期間為2016年7月15日至7月24日。運用Gordon十一項功能性健康型態做為評估指引，透過身體評估、觀察、會談及病歷紀錄等方式收集資料，確立健康問題有：嘔吐、睡眠型態紊亂、無望感等健康問題。照護過程以症狀護理，口腔清潔、選擇清淡、少量多餐進食，教導規則運動、營造睡前環境，運用耳穴按壓療法，改善噁心、嘔吐、睡眠障礙。個案因癌症復發對治療療效產生不確定感及化療後副作用造成身體不適，鮮少與人互動，經常嘆氣、情緒低落，失去治療的信心，產生無望感，藉由主動關懷及病友間的鼓勵，重新啟發生命的意義，依個人的信念，提供另類治療選擇，運用專注冥想，引導身心放鬆及心靈平靜，減輕疾病造成的心理壓力及衝擊，提升自我的價值與期望。建議護理人員在照護類似個案時，運用耳穴按壓，減輕癌症治療的症狀困擾，提升照護品質，增進對治療的信心。

This article discusses the nursing experience about a recurrent ovarian cancer patient who faced huge changes after chemotherapy and felt despairing in cancer treatment. Nursing period was from July 15 to July 24, 2016. The Gordon 11 functional method was applied. Through the evaluation of body, observation, interview and medical records, the patient's health issues were confirmed, these included vomiting, sleeping disorders, bodily changes, and hopelessness caring complication. Nursing methods included symptoms care, oral cleaning, light diet, small amount of meals frequently, regular exercise, establish bedtime atmosphere, and ear acupoints massage to relieve those symptoms. Due to the recurrent cancer and those side effects caused by chemotherapy, the patient felt despair and uncertain. Therefore, the patient did not interact with others, sigh frequently, emotionally distressed, and lost confident in the treatment. Initiative caring and encouragement from other patients provided some impact, advises about rediscovery for the meaning of live, and mediation were also provided to relaxation guidance, peaceful mind, relieved the pressure and impact caused by the illness, and hence improving self-value and belief. This article suggests that ear acupoint massage could be applied to ease symptoms, improve the care quantity, and improving the confidence towards treatment when nursing similar patients.
