一位二次顱內動脈瘤破裂病人之加護病房照護經驗
Nursing Experience from a Secondary Intracranial Aneurysm Rupture Patient in the Intensive Care Unit
摘要
本文敘述一位二次顱內動脈瘤破裂病人之加護病房照護經驗。筆者於2017年3月9日至4月10日藉由直接照護、觀察及治療性會談進行生、心、社會及靈性之整體性評估。確立個案有顱內調適能力降低、身體活動功能障礙及焦慮，且案妻有照顧者角色緊張之健康問題。筆者透過護理照護措施及藥物穩定個案顱內壓、組織灌流與生命徵象；提供早期活動措施與配合復健計畫預防個案關節攣縮。對個案與案妻透過陪伴、同理、傾聽及護理衛教，改善個案對疾病預後不確定性的焦慮情緒，及案妻因個案病況急遽變化產生的緊張不安。護理過程中建立良好且互信的護病關係，最終個案及案妻在醫護團隊協助下，達到心理狀態平衡，以正向且積極態度面對後續復原之路。

This article describes the nursing experience from caring a secondary intracranial aneurysm rupture patient in the intensive care unit. Between March 9 and April 10, 2017, the nurse conducted the holistic assessment of patient's physical, psychological, social and spiritual requirement through direct care, observations, and therapeutic interviews. The result diagnosis patient with declining intracranial adaptive capacity, impaired physical mobility and anxiety health related problems as well as patient's wife had caregiver's role strain generated health problems. For early mobilization interventions and assisting the rehabilitation plan to prevent patient's joints contracture, the nursing interventions and the drugs were given to stabilize intracranial pressure, cerebral perfusion pressure and patient's sign of life. Furthermore, the nurse accompany, listening to both patient and his wife and provide nursing health education to reduce the anxiety of patient from uncertain prognosis of disease and the strain emotion of patient's wife from patient's condition switch rapidly. In the nursing process, the nurse established a proficient and mutual treatment relationship with patient and patient's wife. After the support of medical team, patient and patient's wife finally attain psychological stability and face the path of follow-up recovery with positive attitudes.
