運用跨團隊合作降低呼吸器相關肺炎感染率之專案

本專案於兩個內科加護病房單位，在2015年7月推行VAP照護模式後，7-12月VAP感染率（千分之2.15），超過同年VAP閾值（千分之0.7），引發專案成立動機。運用跨團隊合作組合式照護模式，擬定措施策略，藉舉辦在職教育、建立Moodle教學平台、制定稽核制度、標竿學習床頭點滴架，且立即性的回饋機制，實施後VAP發生率為（千分之0）。本專案結果可平行推廣加護病房，以提升照護品質。

After the VAP care model was implemented in two Intensive Care Units in July 2015, the VAP infection rate from July to December was (2.15 per mille), exceeding the VAP threshold of the same year by (0.7 per mille), so it motives this project establishment。The project uses a cross-team cooperative care model to formulate measures and strategies, by organizing on-the-job education, setting up the Moodle teaching platform, formulating an auditing system, benchmarking bedside drip stand, and an immediate feedback mechanism to improve the incidence of VAP The VAP incidence rate became (0 per mille) after project implementation. The results of this project can be extended to intensive care units in parallel to improve the quality of care.
