運用周全性老年評估技巧照顧一位居家骨質疏鬆症之個案管理經驗

摘要

本文描述照護及管理一位70歲、初次罹患骨質疏鬆，因不識字且服用藥物不當；對疾病治療不確定感的焦慮的特性。個案因感到身體疼痛由門診就醫後，罹患骨質疏鬆症。本案護理期間為2017年6月7日至9月27日，運用老年周全性評估，確立有：知識缺失、潛在危險性跌倒、焦慮等三項健康照護問題，依據個案健康問題設定照護目標，提供居家環境改善、預防跌倒相關認知及運用圖片製作簡易衛教單，以回覆示教了解個案吸收情形。運用陪伴和鼓勵個案表達情感及轉移注意力方式，減輕患者焦慮情形。配合規律回診及衛教介入，提升個案對疾病的認知及自我照顧能力。筆者希望透過老年周全性評估及個案管理照護經驗的分享，提供護理人員照護相關個案之參考。

This article describes the care and management of a 70-year-old patient suffering from osteoporosis for the first time. Owing to illiteracy and characteristic of anxiety and improper use of medication, the case suffered from physical pain and went to OPD and got diagnosis of osteoporosis. The nursing period in this case is from June 7 to September 27, 2017, utilizing comprehensive geriatric assessment and establishes some health problems such as lack of knowledge, potential risk of falls, and anxiety. Setting caring goals based on individual health issues, provide home improvement, preventing falls, and use pictures to make simple health education leaflets for educational purpose. Apply companionship and encourage individual cases to express emotions and divert attention to reduce patient anxiety. Cooperate with regular referrals and health education interventions to improve the patients' awareness of the disease and self-care ability. The author hopes by sharing this case management and care experience through comprehensive geriatric assessment, it will provide a reference for nursing staff to care for relevant cases.
