一位青少女未婚懷孕接受終止妊娠之護理經驗

本文主探討一位青少女未婚懷孕接受終止妊娠之護理經驗。照護期間2019年03月29日至03月31日，藉由直接照顧、觀察、傾聽及會談技巧等方式收集資料，依Gordon十一項健康功能型態進行整體性評估，確立個案有焦慮／與引產過程不瞭解有關、急性疼痛／與引產藥物引發子宮收縮有關、哀傷／與終止妊娠導致胎兒死亡有關、特定知識缺乏／對性觀念與避孕方法不瞭解有關。在照護過程中，運用全程陪伴及關懷等方式建立良好的護病關係，主動提供引產過程之相關訊息以減輕其焦慮；指導使用放鬆技巧及非藥物性處置減緩疼痛；在胎兒娩出後，引導個案抒發內心的感受，接納其哀傷情緒；搜尋相關可利用的資源，提供避孕措施，並增進其自我保護能力，避免再次懷孕。希望藉此護理經驗的分享，提供護產人員照顧此類個案之參考。

This article described the nursing experience of caring an unmarried pregnant teenager to undergo termination of pregnancy. During the nursing period from March 29 to March 31, 2019, data were collected through direct care, observation, listening and interviewing techniques. According to Gordon's eleven health functions, a holistic assessment was conducted to establish that the case was anxious: related to not understanding the process of labor induction; acute pain related to uterine contractions caused by drugs for induction of labor; grief related to fetal death due to termination of pregnancy; lack of specific knowledge related to sexual behavior and contraceptive methods. In the process of nursing, applying the complete process of companionship and care to establish a good nursing relationship, and actively provide information about the induction process to alleviate the anxiety; guide the usage of relaxation techniques and non-drug treatment to relieve pain. After the delivery of the baby, guide the case express her inner feelings, accept her grief; search for relevant resources that are available, provide contraceptive measures, and enhance their self-protection ability to avoid undesired pregnancy again. Hopefully by sharing this nursing experience will provide reference for the nursing staff when taking care of similar cases.
