降低急診室血液培養檢體染污率之專案

Project of Lowering Contamination Rate of Blood Culture Specimens at Emergency Department

摘要


護理人員的採檢過程是影響血液檢體品質的最主要原因。2016年7月1日至12月31日,本單位護理人員血液培養採檢技術正確率僅達65.6%,汙染率約佔3.7%,超出標準閾值3%甚多,進而影響病人疾病治療進度。經現況之分析原因包括:護理團隊(含專科護理師)對於血液培養檢體採集無菌觀念不足、缺乏血液培養檢體採集教育訓練、相關採集設備不足、缺乏定期稽核機制。藉由單位修、制訂採檢標準技術標準流程及稽核表、安排在職教育及新人訓練、全面改用2% Chlorhexidine gluconate消毒溶液、增設採檢輔助工具等措施改善後,血液培養採檢技術正確率提升為95.6%,汙染率降至1.3%,以達到提供病人安全之最佳照護品質。

The quality of blood sample is predominantly influenced by the inspection process of nursing staff. During July 1st to December 31st of 2016, the correct percentage of blood culture sample was merely 65.6% and the contamination percentage was 3.7% in our department. It exceeded 3% of the standard threshold level, hence it affected the plan of patient's treatment progression. After analysis, we found the reasons were insufficient sterile concept of nursing team (including nursing specialist), lack of education and training for blood culture specimen collection, insufficient relevant collection equipment, and lack of regular audit mechanism. The accuracy rate of blood culture collection and testing technology has improved through measures such as unit revision, development of collection and inspection standards, technical standard procedures and audit forms; arrangement of on-the-job education and training for new staff, comprehensive application of 2% Chlorhexidine gluconate disinfectant solution, additional collection and testing aids. It improved to the rate of 95.6%, and pollution rate reduced to 1.3%, for the purpose of achieving the best quality of care to provide patient safety.
