提升肺臟移植團隊護理人員執行微創血液動力學監視照護完整率

Enhancement of the Complete Rate of Minimally-Invasive Hemodynamic Monitoring and Care Provided by Nurses in the Lung Transplant Team

摘要


肺臟移植術後重症照護期間,血液動力學不穩定,故血液動力學監測至關重要。本專案主旨提升肺臟移植團隊護理人員執行微創血液動力學監視照護完整性,現況分析顯示本單位肺臟移植團隊護理人員執行微創血液動力學監視照護完整率僅58.9%,照護認知僅58.8%。原因為儀器組裝與設定生疏及對參數判讀認知不足等。專案目的為提升肺臟移植團隊護理人員執行微創血液動力學監視照護完整率達92%。改善措施為製作儀器操作與標準照護手冊、教學影片、床邊小卡、模擬情境教學等。實施後護理人員微創血液動力學監視照護完整率由58.9%提升至98.2,照護認知達97.6%。專案目標達成,顯示此專案強化肺臟移植團隊護理人員進行微創血液動力學照護認知及技術之能力,亦提升臨床重症照護品質。

During intensive care after lung transplantation, hemodynamics is still unstable, so hemodynamic monitoring becomes critical. The purpose of this project is to improve the complete rate of minimally-invasive hemodynamic monitoring and care provided by nurses in the lung transplant team. The current situation shows that the completion rate of hemodynamic monitoring and care provided by nurses in the lung transplant team was 58.9% and cognition of hemodynamic monitoring merely 58.8%. The issue identified included unfamiliarity with equipment configuration and operational procedure as well as insufficient knowledge about parameter interpretation. Therefore, this project was established to improve the care integrity rate of the lung transplant team nurses performing minimally invasive hemodynamic monitoring aiming to reach completion rate of 92%. Improvement measures include producing manuals for equipment operation and standards of care, demonstration videos, bedside cards, and simulated situational teaching. The results revealed that the completion rate of hemodynamic monitoring and care provided by the nursing staff increased from 58.9% to 98.2%, cognition rate of hemodynamic monitoring reached 97.6%. This achievement reached the goal of the project, demonstrates this project strengthens the ability of the lung transplant team nurses when perform minimally invasive hemodynamic care cognition and technical capacity, and hence improves the quality of clinical intensive care.
