住院老人跌倒評估、處置與預防

Assessment, Treatment and Prevention of Falls in Hospitalized Elderly Patients

摘要


隨著病人安全意識抬頭,預防住院病人跌倒已是全國重要議題之一,老人泛指年齡大於65歲以上長者,一但發生跌倒,造成的傷害可能是輕微至嚴重不等,為此本文透過文獻回顧,歸納住院老人跌倒危險因子包含:行為或心理、生物、環境或情境及社會經濟等四因素,並介紹「住院老人跌倒風險篩檢及危險因子評估以及住院老人跌倒預防措施」,透過風險篩檢辨識出高跌倒風險的老人,並藉由危險因子評估瞭解高風險原因,評估出危險因子後提供對應護理措施來預防跌倒,藉此期能提供臨床人員在執行預防住院老人跌倒計畫之參考,以降低住院老人的跌倒發生率,提升病人的安全。

The prevention of the inpatients' falling off has become one of the important issues in the country as the rising awareness of patient safety. Seniors generally refer to the elderly people aged over 65 years. Once a fall occurs, the injuries caused may range from mild to serious conditions. Therefore, this article reviews the literature and concludes that the risk factors for falls of the hospitalized elderly include: Behavioral or Psychological; Biological, Environmental or Situational; and Socioeconomic Factors. This essay introduced: "Fall Risk Screening, Risk Factor Assessment for Hospitalized Elderly and Fall Prevention Measures for Hospitalized Elderly". Identify the elderly with a high risk of falls through risk screening, understand the reasons for the high risk through risk factor assessment, and hence provide corresponding nursing measures to prevent falls after assessing the risk factors. This case hopefully can provide a reference for clinical staff to implement the fall prevention plan for the elderly in hospital, in order to reduce the incidence of the elderly falls in hospital and improve the safety of patients.
