本文探討一位罹患高熱感染相關性癲癇症候群男童之護理經驗,照護期間為2019/11/20-12/20。運用整體性護理評估,透過觀察、會談、身體評估及直接照護等方式蒐集資料,確立健康問題有呼吸道清除功能失效、身體活動功能障礙、照顧者角色緊張。照護期間密切監測個案生命徵象、呼吸型態變化、癲癇發作情形,給予適當的護理處置減少合併症發生。當面對個案主要照顧者,運用同理心,適時陪伴、傾聽,增加照顧者自信心,鼓勵說出內心感受,提供抒發管道、給予適當的支持,提升對於個案罹病後照護的正向感。期望藉由本經驗分享,可提供罹患FIRES個案或類似個案護理之參考。
This article discusses the nursing experience of a boy with Febrile Infection-Related Epilepsy Syndrome (FIRES) and his caregivers in the pediatric intensive care unit. The nursing period was from November 20th to December 20th of 2019. By applying holistic nursing assessment, the author collected data through observation, interviews, physical assessment and direct care, to identify health problems such as respiratory clearance failure, physical activity dysfunction, and caregiver role stress. During the care period, the patient's vital signs, changes in respiratory patterns, and epileptic seizures are closely monitored, and appropriate care is provided to reduce the chances of comorbidities. When facing the case's main caregiver, utilize empathy, accompany and listen in a timely manner, amplify the caregiver's self-confidence, encourage them to express their feelings, provide channels for expression, provide appropriate support, and enhance a positive sense of care for the patient after the illness. Hopefully through this experience sharing, we can provide a reference for the care of patients suffering from FIRES or similar cases.
