Nursing Experience for Caring for A Toddler with Kawasaki Disease

摘要

本文描述一位川崎氏症幼兒因高燒且食慾不振入院治療之護理經驗。照護期間自2018年10月12日至2018年10月20日，運用羅氏適應模式為評估工具進行整體性的評估，確立健康問題有口腔黏膜障礙、體溫過高、照顧者角色緊張。引導個案願意配合口腔護理，護理過程中筆者給予案母口腔潰瘍飲食的注意事項，教導發燒時給予適當的飲食及液體的攝取，以主動積極關懷及個別性的護理照護，並能獲得完整性的照護。建議可安排醫病共享決策，透過醫療團隊會議，共同擬定護理計劃，提供相關病程資訊及持續性追蹤，以提升出院後主要照顧者居家照護的信心。

This article describes the nursing experience of a toddler with Kawasaki disease who was admitted to the hospital for high fever and loss of appetite. During the care period, from October 12, 2018 to October 20, 2018, Roy's adaptation model was used as an assessment tool. A holistic assessment was conducted of the health problems, including oral mucosal disorders, and hyperthermia, and caregiver's role strain. Many strategies of improvement were adopted. Suitable guidance was provided to the patient to cooperate with oral care. During the nursing process, a diet was provided to the mother for oral cavity ulcer. We taught the mother to provide an appropriate diet and manage fluid intake during the toddler's fever. Active care with individualized nursing was provided specifically for the case. It is suggested that a shared decision making can be arranged between the medical staff and patient. Through medical team meetings, joint formulation of the nursing plan, providing relevant disease course information and continuous tracking, nurses can enhance the confidence of main caregivers after discharge.
