糖尿病患下肢傷口癒合不良之病例報告

摘要


本案為一位四十三歲男性患者,有高血壓及糖尿病史,規律服藥但數值控制不佳,在2019/04/22發現右小腿脛骨內側紅腫、局部疼痛且高燒至39°C,求治風濕免疫科診斷為蜂窩性組織炎,服用Augmentin一週後紅腫範圍仍逐漸擴大,並伴隨間歇性發燒,故於2019/04/29收置住院治療。患者就診中醫已間隔兩個月,而傷口遲遲未能癒合,呈現紅腫熱及按壓疼痛,且周圍皮膚轉為紫暗色,經濃縮中藥粉劑及針灸治療後,瘡口紅腫消退,疼痛減緩並收口癒合。

This is a case of a 43-year-old male patient with a history of hypertension and diabetes. He took medication regularly but the numerical control was poor. On 2019/04/22, he found his medial shin of the right calf red and swollen, with local pain and a high fever of 39 °C. He went to the rheumatism immunity department for help and was diagnosed with cellulitis. After taking Augmentin for a week, the range of redness and swelling continued to expand gradually, accompanied by intermittent fever, so he was admitted to hospital on 2019/04/29. It had been two months since the patient sought help from a TCM doctor, with an unhealed wound, red, swollen, hot and pressing pain, while the surrounding skin turning dark purple. After treatment with concentrated Chinese medicine powder and acupuncture, the redness and swelling of the sore subsided, the pain relieved, and the wound healed.
