International Perspective of Hospice Palliative Care Development and Its Inspiration on Policy in Taiwan

摘要

有鑒於國內老年人口快速上漲，安寧緩和療護需求日益增加，本文藉由文獻回顧，闡述近年國際間安寧緩和療護之發展趨勢及面臨挑戰，彙整歸納歐美等發展歷史較久且完善國家之經驗，供主管機關與相關專業人員共同深思參考。總結如下：安寧緩和療護對象已不限於癌症病人，尚包括其他各種疾病之臨終病人，尤其是老人；在發展與給付政策方面，應配合長期照護系統之規劃；照顧者支持系統與哀傷輔導為安寧緩和療護中不可獲缺的一環，應盡快建置制度供給需要的民眾；妥善訓練運用志工做為一重要資源；基礎生死教育與醫學教育應予以強化；儘早思考如何面對安寧緩和療護之挑戰。

The demand of hospice palliative care is growing because population in Taiwan is aging. This review article illustrates the international perspective of hospice palliative care development and challenges they faced. Value experiences in the America or Europe countries which have longer history and integration are summarized for government and related professionals to consider. The conclusions are as follows. Patients received hospice palliative care are not only suffer from cancer but also varies diseases, especially in the elderly. The development and payment policy of hospice palliative care should be coordinated with long term care system. Moreover, caregiver and bereavement support is necessary and should be offered systematically. Well trained volunteers will be an important human resource. In addition, the death education should be enhanced. In the end, how to face the challenges of hospice palliative care should be considered as early as possible.
