摘要

藥物治療評估（Drug Regimen Review, DRR）是針對諮詢藥師在長期照護機構內進行專業服務的一個專有名詞。藥物治療評估不是調劑處方箋，而是針對醫師開列處方箋準備調劑前的或已調劑好的、住民正準備服用的或已服用的藥品，藥師來評估用藥之適當性與安全性。藥物治療評估是由負責長期照護的諮詢藥師來執行，不論是處方箋調劑前在藥局內或之後在長期照護機構內的某個時間點進行評估。此評估之目的在確保病人的藥物治療能達到最佳之治療結果，這包括考慮到病患每一個用藥之適應症、有效性及安全性，以及病人對這些藥物治療的配合度。因此，諮詢藥師的主要功能在發現、解決，以及預防藥物治療問題。本準則在協助評鑑人員來評值藥師針對個別住民執行藥物治療評估之品質，並確認改善服務流程的方法。

Drug Regimen Review is a specific term for pharmacist professional service conducted in the long-term care facilities, and is not a prescription dispensing. It is focus on the drugs physician prescribed (prior to dispensing/ after dispensing but prior to patient taking/ after patient taking) to have pharmacist evaluating its appropriateness and safety. Drug Regimen Review is implemented by corresponding long-term care pharmacist, whether conducted prospectively in the pharmacy and/or concurrently at the long-term care institution. The purpose is to assure the drug therapy can reach the goals of therapy, including every drugs to have its corresponding indication, is effective, safe and convenient for patient to use. The mail role of the consultant pharmacist is to identify, resolve and prevent any drug therapy problems. This guideline is to assist the accreditation process to evaluate the quality of drug regimen review conducted by the consultant pharmacist and to improve the process.
