A 22 year old man presented to our emergency department with a history of pain in his left abdomen and left loin. He had chillness, but no fever. On examination he had left loin knocking pain. Urolithiasis was impressed then. His urine analysis was positive for blood. Tracing his past history, he often suffered from urinary tract infection. The urinary tract abnormality was highly suspected. Intravenous urogram was arranged in the emergency room. On the urogram films left hydronephrosis, left ureteric calculus, and intravesical ureterocele were seen. A typical cobra head appearance characteristic of a ureterocele was demonstrated. Clinical presentation of a ureterocele varies considerably. Patients commonly present with recurrent urinary tract infection, ureteric calculus and nephropathy..
