摘要

本研究針對1988-1997十年間，發表於國內健康專業學術性23種雜誌中，以居家護理業務為探討主題之研究文獻。探討其一般概況（研究論文篇數、發表之專業性雜誌、研究主題及作者背景資料）並進行研究文獻分析（研究目的與研究架構陳述之有無、研究設計種類、資料收集方法、研究工具之信效度種類、使用之統計方法）。經由地毯式搜尋，共搜尋到28篇文獻，以發表於護理專業性雜誌最多，計57.1％。文獻主題顯示1990-1994間以成本與效益為主，1996年始主題具多樣性。作者背景以居家護理臨床工作人員為多（42.9％）；從事護理專業佔78.6％；75.0％的文獻由多位作者合著。扣除個案研究報告8篇，20篇皆清楚陳述研究目的；惟僅35.0％引用架構；研究設計以量性為多，且65.0％為描述性研究設計。資料收集以問卷法最多（60.0％），次為文獻記錄分析法（50.0％）說明研究工具之信度者為57.9％，說明效度者為42.1％。19篇量性研究中，63.2％合併使用描述性與推論性統計。分兩階段（1988-1992與1993-1997）比較前後五年的狀況，顯示1993-1997之文獻篇數顯著增加、主題具多樣性、合著人數多、信效度報告有改善、但引用架則未見改善。作者與過去相似文獻之發現做比較、探討居家護理學術發展軌跡並嚐試分析影響居家護理研究執行與發表之因素；本研究結果可做為居家護理研究之參考。

Home care nursing program in Taiwan has been developed since 1987. This study seeks to analyze the home care nursing research published from 1988to1997 in Taiwan. Twenty three health professional journals were reviewed and 28articles (including case study) were found, of which 57.0% were published in nursing journals. Between 1990and 1994, research was focused on cost effectiveness, but since 1996 has diversified. 42.9%of the first authors were home care nurses and worked in the nursing profession (78.6%). Seventy five percent of the articles had a co-author. Excluding case studies, the remaining 20 articles all had purpose in research, of which 35.0% presented a framework for guiding research, 57.9% addressed reliability and 42.1%addressed validity. Nineteen articles were quantitative research design, and collected data using questionnaires (60.0%) and analysis of clients’ records (50.0%). Sixty three percent of the quantitative articles combined descriptive and inferential statistic analysis. The analysis of articles published in two stages (1988-1992,1993-1997), showed that the number of articles, diversity of topics, coauthored, validity reports and reliability reports had all increased between 1993and 1997. The findings of this study could be used as a reference to improved further research in home care nursing.
