摘要

隨著老化人口的增加，罹患退化性關節炎患者也隨之增多，而至目前為止，人工關節置換術是有效的終極治療方法。台灣髖關節置換術患者在病例計酬的保險給付下，術後感染、脫臼及30天再入院等比率均較其他國家高。環視今日的醫療環境：醫療費用高漲、保險給付改變、健保論病例計酬制度施行等，迫使病患住院日數必須縮短，這也使得病患術前的衛生教育計畫和術後的復健活動指導顯得格外重要。衛生教育可增加自我效能，進而能促進手術康復的情形。然而，目前國內尚無完整的專業護理指導教材提供病患，故本研究旨在研發一套髖關節置換術護理指導的多媒體影音光碟與手冊。此研發完成的光碟與手冊乃是利用多媒體影音系統來提供文字、動畫、圖片、影像及聲音等多元性的指導，如此可以期望能提供專業者的教學資源。另外，此光碟與手冊亦可提供病患及家屬自學照護的需求，以期提高自我效能與功能性活動，並期望幫助髖關節置換術患者儘快達到最佳的復原狀態。

Along with aging process, the incidence of osteoarthritis has been increased. Until now, artificial joint replacement is the final treatment for such degenerative joint disease. With the case payment insurance policy, patients with artificial joint replacement in Taiwan had obvious higher post-surgical infection rate, dislocation rate and 30-day readmission rate than the ones in other countries. Because of the increasing medical expenses, various changes in insurance reimbursement, and the implementation of case payment scheme, the length of stay in hospital shortens. Thus, providing patients with proper education for pre-surgery preparation and post-surgery rehabilitation has become more important. Based on the literature, health education can effectively improve self-efficacy to regain health and facilitate the rehabilitation after surgery. The project is to develop and test the multimedia VCD and printed nursing guides owing to the deficiency of above educational instruction. This multimedia-based instruction is developed by the multimedia videodisc system to provide words, animation, figures, images, sound, etc. The multimedia VCD and printed nursing guides can improve the learning effect of the patients and their families for better recovery.
