一位費力型中暑個案之護理經驗
Nursing Experience in a Patient with Exertional Heat Stroke
摘要
本文主要探討一位剛入伍新兵，在例行的1000公尺跑步訓練後，發生費力型中暑，並導致多重器官損傷的護理經驗。護理期間自95年7月7日至95年7月17日，筆者運用重症護理評估模式，藉由身體檢查、會談、觀察、查閱病歷等直接及間接方式，確立個案有：「體溫過高」、「高危險性損傷」、「焦慮」等護理問題。擬定重症照護計畫，於急性期主要以穩定生命徵象、維持身體正常機能、降低器官衰竭併發症發生為目標，使個案在轉出加護病房時，因中暑導致的多重器官損傷病情獲得明顯改善，且不影響其生理機能。經由整體性護理過程及持續性復健計畫，個案於出院時可回復日常生活功能。希望藉由本篇報告，提供臨床實務作參考。

This paper described the experience of nursing a patient who experienced exertional heat stroke after undergoing a routine running training of 1,000 meter and subsequently developed multiple organ failure. During the period of nursing care, from Jul. 7, 2007 to Jul. 17, 2007, data were collected through direct and indirect ways including physical assessment, interviews, and observation. The health problems identified included ”hyperthermia”, ”injuries of high risk”, ”anxiety”. After the implementation of a nursing care and rehabilitation plan, the patient was able to resume his daily life functions when he was discharged from the hospital. It is hoped that findings from this report can be applied to clinical nursing practice.
