運用Watson關懷理論妤燒炭自殺患者之加護照護經驗

Application of Watson's Theory in Caring a Charcoal Burning Suicide Patient in an Intensive Care Unit

摘要


本文係運用Watson關懷理論,協助一位中年男性因失業、債務及與家人溝通產生障礙,於無處可尋之下以燒炭自殺方式逃離壓力的護理經驗。筆者於2011年7月1日至7月6日以Gordon十一項健康功能型態評估,確立個案有氣體交換障礙、個人因應能力失調及家庭因應能力失調等健康問題。運用Watson關懷理論為依據,採主動關懷的態度建立良好信任關係,引導個案正視自殺的導因,給予心理支持,增進正向的因應策略,鼓勵與家人互相表達內心感受,改善彼此的互動關係,另提供良好的社會資源及轉介,藉由電訪提供持續性照護,使個案能主動尋求支援,透過家屬及各醫療團隊的合作下,協助個案恢復健康,重拾信心並發展正向的調適技巧。

This article describes an experience with the application of Watson's theory in caring for a helpless middle-aged man, who committed suicide by burning charcoal due to stress from debt, unemployment, and communication problems with the family. The nursing period was from July 1 to July 6 in 2011. Gordon's 11 functional health patterns were applied to evaluate the patient's condition and three nursing diagnoses were identified as impaired gas exchange, ineffective individual coping, and ineffective family coping. By adopting Watson's theory, a trusting relationship with the patient was established through caring. The patient was guided towards facing the causes of his suicide and given psychological support to help form positive stress coping strategies as well as improving the interaction with the family by expressing inner feelings. Also, he was given great referrals and social resources. The patient was able to reach out for support through the continuous care via telephone. With the help of his family and medical teams, the patient regained his health and confidence, thereby developing healthy coping skills.
