社區安寧照護之未來發展

摘要

台灣已將社區安寧照護列入健保給付之範圍，期望透過優質的居家安寧照護服務，讓病人在宅得到善終。衛福部於2017年推行長期照顧十年計畫2.0，透過多目標社區式支持服務、出院準備、居家醫療轉銜在宅臨終安寧照護為重要的服務項目。要成功推展社區安寧照護有賴於：（1）醫療照護人員安寧療護知能提升，適時引導病人及家屬及早進入安寧照護之決策思考；（2）及早制定末期病人安寧照護計畫；（3）長照機構建立安寧照護服務模式；（4）預立醫療自主計畫之推動與落實。

Community palliative care has included in Taiwan health insurance payments. It is expected that through the high quality palliative home care, the patients will get good death at home. In 2017, Taiwan promotes 10-year long-term care 2.0 plan, through multi-target community-based support services; discharge planning, home health care, and palliative care at home as an important services. To be successful in promoting community palliative care depends on: (1) to improve palliative care knowledge and ability for health care providers, timely guidance of patients and their families early into the palliative care decision-making; (2) to develop the patient palliative care program in early stage; (3) to establish a palliative care service model in LTC setting; (4) to promote and implement advance directives for the people.
