Improving the Execution Rate of Cardiac Rehabilitation Care in Patients with Cardiac Failure

摘要

心臟衰竭病人接受心臟復健後，可改善運動耐力與心臟功能，提升生活品質、降低死亡率。因臨床發現並非每一位須接受心臟復健的心臟衰竭病人接受合宜的復健照護，故引發改善動機。專案小組於2019年3月20日成立，經現況調查發現本單位心臟衰竭病人接受心臟復健照護執行率僅60.6％，故本專案的目的為提升心臟衰竭病人接受心臟復健照護執行率至91％。經由專案設置心臟衰竭復健提醒會診小卡及口訣、心臟衰竭復健會診連結、舉辦護理師心臟衰竭復健衛教工作坊、設計簡易型心臟衰竭病人心臟復健衛教單張。專案實施後，除了提升醫護人員對於心臟復健的認知，也改善心臟衰竭病人照護品質，並提升心臟復健衛教滿意度，結果顯示心臟衰竭病人接受心臟復健照護執行率由60.6％提升至93.3％，達到專案目標。

For patients who suffer from cardiac failure, going through cardiac rehabilitation may enhance their exercise tolerance and cardiac function. Patients who suffered from cardiac failure did not receive sufficient cardiac rehabilitation care, which triggered the motivation for improvements. The project team was established on 20 March 2019. As confirmed by the previous investigation, the execution rate of cardiac rehabilitation care was low as 60.6%. Therefore, the purpose of this project was to increase the execution rate for cardiac rehabilitation care to 91%. Through the project, a small card and memory trick as reminders for cardiac rehabilitation were established. Workshops regarding cardiac rehabilitation education was held, and a simplified health education flyer was designed for patient education. After the implementation of the project, it enhanced nurse awareness of cardiac rehabilitation, improved the quality of care for patients with cardiac failure, and increased the satisfaction of cardiac rehabilitation education. The results show that the execution rate for cardiac rehabilitation care increased from 60.6% to 93.3%, achieving the project's goal.
