手握式測力器於台灣東部原住民女性老年人肌力之再測信度-以花蓮縣秀林鄉某社區部落為例

Dynamometer in Aboriginal Elderly Women in Eastern Taiwan. A Preliminary Report from Hsiu-Lin Township in Hualien

背景與目的:人口老化是台灣重要的社會議題,老化所帶來各項生理機能下降,肌力衰退是其中之一。肌力衰退和老人日後生活功能獨立性及跌倒與否有顯著關係。方便且精確地測量老人肌力有其重要性。然而,過去較少針對原住民老年人肌力進行再測信度的研究。本篇研究目的為利用手握式測力器探討原住民社區部落女性老年人肌力的測試者內和測試者間再測信度。方法:以方便取樣收集花蓮縣秀林鄉某原住民社區部落之女性老人為受試者。第一階段收集16位女性原住民老人進行同日測試者間再測信度。由一位治療師使用手握式測力器以Break方式進行雙側上、下肢各五組肌群測試,每回測兩次,間隔30秒。休息30分鐘後再由第二位治療師以相同方式進行測試。第二階段由第一位治療師另外再收取15位受試者進行隔週測試者內再測信度,兩回測試間隔一週。結果:第一階段測試者間再測信度之級內相關係數(Intraclass correlation coefficient, ICC)值為0.907至0.987之間,標準誤為0.28至1.64公斤,最小可測得改變為0.79至4.54公斤,變異係數為3.04%至9.43%。第二階段測試者內再測信度之ICC值為0.911至0.977,標準誤為0.63至1.63公斤,最小可測得改變為1.75至4.51公斤,變異係數為5.48%至9.31%。結論:本篇研究顯示以手握式測力器對於東部原住民社區部落女性老人上、下肢肌力均具有良好之測試者間與測試者內再測信度。未來研究建議可探討不同地域族群或健康狀況以了解其再測信度的結果。

Background and Purpose: Aging is one of top concerning issues in Taiwan society in recent decades. Reduced muscle mass and declined muscle strength due to aging not only poses a great impact on independence of functional activity in elderly but is the major cause of fall. To effectively and precisely evaluate the muscle strength in the elderly could make a great help in designing their rehabilitative programs later. However, little research was studied the test-retest reliability of muscle strength using hand-held dynamometer in aboriginal elderly in Taiwan. The purpose of the study was to investigate the test-retest reliability of muscle strength with handheld dynamometer in upper and lower extremities of community-dwelling aboriginal elderly women. Method: A convenience sample of 31 aboriginal elderly women from Hsiu-lin township, Hualien County were recruited in two phases of the study. In the first phase, sixteen elderly Taiwanese aboriginal women were collected to calculate the inter-rater test-retest reliability of muscle strength. The first therapist using hand-held dynamometer with "Break" method measured twice the strength of bilateral upper-extremity and lower-extremity muscle groups, with interleaving 30 seconds between each muscle testing. After another 30-minute rest period, the second therapist performed the same muscle-testing procedure as the first one. In the second phase for intra-rater test-retest reliability, another 15 subjects were recruited and conducted the same procedure as the first phase by the first therapist, but one week apart from the two measurement sessions. Results: In the first phase, the inter-rater intraclass correlation coefficients (ICCs) were ranged from 0.907 to 0.987, with standard error of testing ranged from 0.28-1.64 kg and coefficient of variation from 3.04% to 9.43%. In the second phase, the intra-rater ICCs were ranged from 0.911 to 0.977, with standard error of testing ranged from 0.63-1.63 kg and coefficient of variation from 5.48% to 9.31%. Conclusion: Both intra-rater and inter-rater ICCs were high in bilateral upper-extremity and lower-extremity muscle groups with hand-held dynamometer testing in the elderly Taiwanese aboriginal women. Further investigations would be warranted for a larger sample with different sex or health status to obtain more information about the intra-rater and inter-rater reliability of hand-held dynamometer measurements.
